Easiest would be to simply form a chord and move the pick along barely pausing on the next string making sure each note sounds out, yet keeping a continuous flow. The first "real" sweep I learned was something like…

    ———————–7h 10p 7
    ——————–7- – - – - – - -7
    ————-6 / 9 – - – - – - – - – - 9 \ 6
    ———-7- – - – - – - – - – - – - – - – - – 7
    ——-7- – - – - – - – - – - – - – - – - – - – - 7
    5h 9 – - – - – - – - – - – - – - – - – - – - – - – - 9p 5

mid finger slides 6-9 and back, first and little for hammer-on and pull-off, ring finger on the A and D strings, obviously only picking once for the h, p and slide. Sweeps take a bit of practice to perfect. Get ahold of some of Marty Friedman and Jason Becker’s work, they had a band called Cacophony when they were teens, awesome ultrafast perfect sweeps. That’s about the best I can answer this one.
